Then

1. Please type your information into the application form. Type your information into the gray boxes.

If you cannot get the form to open at home, please fill it out from a campus computer

2. Print the completed form.

3. Take it the business office to pay the $65 application fee.

The business office will stamp your form and staple a receipt to it.

However, if you are receiving FINANCIAL AID, please take your PTK application to the financial aid office.

4. Take the stamped form and drop it off to Mrs. Carter

Mrs. Carter has a desk located in B134 (you must walk thru B130 to get there) or you can put it in her mailbox in the back right-hand corner of B130.
