Steps to Request Tutorial Services 1. Pick up the two forms in the Tutoring Center/Learning Center, Arts & Science Center, Room 336 or download the two forms. 2. Complete and return the two forms to the Tutoring Center. 3. Bring a copy of your most recent 1040 Income Tax Return to the Tutoring Center with the other forms. 4. With the completion of the paperwork, an appointment will be scheduled. 5. Complete the appointment and you will receive tutors' names and phone numbers to contact for tutoring services.
