Steps to Request Tutorial Services 1. Pick up the two forms in the Tutoring Center/Learning Center, Arts & Science Center, Room 336 or download the two forms. 2. Complete and return the two forms to Lawrence Bivens in the Tutoring Center/Learning Center. 3. Make a copy of your most recent 1040 Income Tax Return and bring it to Lawrence Bivens in addition to the forms above. 4. With the completion of the paperwork, an appointment will be scheduled. 5. Complete the appointment and you will receive a tutor's name and phone number to contact for tutoring services.
