| Due to strict licensing requirements for Delaware Technical & Community College Youth Programs, we are obligated by the State of Delaware to have on file current immunization records for all campers attending our camp. Provide us with a photocopy of your child's immunization records show that he/she is up to date on the following immunization shots: |

| |
| By signing the registration and allowing your child to attend you accept the following terms: |
- No reduction in tuition will be made for late arrival or early departure.
- Should my child be dismissed from camp, no part of the tuition will be returned.
- No refunds or transfers.
